2. Provide the name of technical assistance case manager/provider.  This is the name of the person actually performing and/or managing the technical assistance.

3. Describe the scope of services you and your team are charted to supply. (less than 30 words)

4. Describe your proposed technical assistance project.  Provide background information and discuss in as exact terms as possible what you are being asked to do.  (less than 100 words)


5. Describe the team of people assembled to perform the technical assistance activity and the collective background of the members. (less than 30 words)


6. Describe the deliverables, the timeframe and your estimated level of effort to complete the task(s). (less than 35 words)



7. Describe your available resources, including any leveraging that might be available to you.  (less than 30 words)


8. Describe any risks that you can see and how you plan to mitigate them.  (less than 35 words)


9. Describe the concerns or questions that you have with the project and/or any aspect in which you feel you need advice, consultation, or guidance. (less than 100 words)


10. Indicate if you believe follow-on consulting might be desirable or necessary and what form you would like that consulting to take (telephone, face-to-face, etc). (less than 30 words)
